PHC Wellness Program

The PHC Wellness Program builds upon the PHC Access Program by offering extensive wellness counseling, monthly massage therapy, and additional physical medicine as medically indicated:

  • Same day appointments (within 24 hours of request)
  • Appointments with minimal or no waiting time
  • No time limits on visits with your physician
  • 24/7 access to your personal physician via phone and email
  • Coordination of all medical care, including direct consultation with specialists
  • Annual complete medical Physical
  • Monthly Wellness Sessions (12 per year)
    • Wellness consultation with physician/health coach
    • Complete massage and/or physical therapy session
  • Additional physical medicine sessions when medically appropriate
    • Chiropractic care
    • Intensive massage therapy
    • Acupuncture
    • Physical therapy
  • Lifestyle Modification Therapy
  • One Special visit per six months (two per year)*
    • Home visits – Scheduled appointments or after-hours visits in the home
    • Workplace visits – Appointments at the workplace (if within acceptable geographic limit)
    • Emergency Room – Meet Member at the ER to facilitate emergency care
    • Chaperone services – Attend visits with specialists as personal medical chaperone
    • Hospital/Nursing Home Visits
    • Wellness Sessions
    • Appointments for family members or guests in need of unexpected care

* Unused Special visits roll over to the next period.
